A family mourns the loss of a father and husband in New York after a workplace accident took his life.
According to local police reports, the 62-year-old man was working with the state Department of Transportation on a bridge. He was riding in a basket elevated by a boom truck when the truck ran into a steel support structure of the bridge. The man struck his head hard enough to cause fatal injuries; the second man riding with him was lucky enough to remain unhurt in the accident.
The workplace accident is now being examined by several Public Employee Safety and Health Unit investigators as well as DOT engineers. As with any workplace accident, the Occupational Safety and Health Administration is likely to send a representative as well to determine whether the workplace is safe enough to resume operations by fellow employees.

One carnival came to a sad end when a man lost his life in North Carolina.
According to the state's Labor Department, the man was part of a crew disassembling a ferris wheel at the carnival. After investigating the scene of the incident, the North Carolina Labor Department found 12 ways in which the company that ran the ferris wheel had violated state code. Although the company's headquarters is in Georgia, the state of North Carolina will be holding the company responsible for the man's death.
As a result of the incident, the company received fines totaling $27,000. It has 15 days to contest the citations, or work out a plan to make the full payment.
